To speed up your entry to the club please sign this waiver prior to arriving at the NYX 

PLEASE READ BEFORE SIGNING

  1. Everyone entering NYX Lounge MUST sign the waiver.
    1. If you are a couple, be sure to include first and last names for both of you. 
    2. BOTH of you must sign the waiver in the signature space provided.
    3. A valid email must be provided. (a copy of the waiver will be sent to you)
  2. If you have signed the waiver before, you do not need to sign again.
  3. Please be prepared to present ID at the club.
  4. Be sure to agree to receiving emails to not miss our members only specials events, and promos.

NYX is a members only club, we reserve the right to refuse entry.
